Handover

Ms Rebecca Harris

You have been allocated Ms Rebecca Harris, a 32 year old female who has just given birth to her first child baby Benjamin at 13:10 hrs

Past History

Ms Harris has no relevant medical history and has experienced no complications during her pregnancy. Ms Harris has been birthing for approx 9 hours. The birth was a normal vaginal birth and was uneventful. The placenta and membranes were checked, the placenta appeared complete and the membranes were ragged.
Neurologically

Patient is alert and orientated
Vital Signs at 1330

T 36.6 ÂșC

HR 70 bpm and regular

BP 110/70 mm Hg

RR 18


Input/Output


Ms Harris has been commenced on IV therapy (6 hrs ago) at an 8/24 rate via a peripheral cannula due to dehydration

An estimate of 800ml blood loss has been lost vaginally since the birth

Rebecca has not voided for 3 hours. On questioning Rebecca states that she feels the need to pass urine but has been unable to do so. The obstetrician has stated that the patient may be in urinary retention and the bladder is preventing the uterus from contracting. Given this, the insertion of an IDC has been ordered.


As you assess Rebecca, she complains of a gushing sensation between her legs.

Activity


The time is 1400hrs. You have been allocated MS Harris for this shift.

You are required to:

    * Develop, prioritise and implement a plan of care for this patient using the information provided
    * Assess and monitor the patient appropriately
    * Insert an indwelling catheter
    * Administer medications according to the medication chart
    * Document all care delivered
