Week 13 Lab
Day 8
At 0540hrs Mrs. Brown experienced an episode of severe right sided chest pain associated with nausea.
Vital signs:
HR: 108
BP: 127/52
RR: 24
Sp02: 90% on room air
Chest Pain rating: 9/10
The RN immediately calls for the doctor to review Mrs. Brown. The following tests were ordered:
ECG: indicates T wave inversion anterior leads V1-V4
CXR: focal oligoemia, enlarged right descending pulmonary artery, a diaphragm elevation
ABG: respiratory acidosis
D-Dimer: +ve result
Echocardiogram: Right ventricular dysfunction
After reviewing all the above results a diagnosis of Moderate Right Pulmonary Embolism was given. The Doctor orders PO Warfarin and S/C Clexane.
Lab Activity
It is now 1645hrs and you are the registered nurse assigned to Mrs. Brown. You have just helped Mrs. Brown onto the bedpan to open her bowels. Before you leave the room to give Mrs. Brown privacy she goes pale and slumps forward. You recognise from the cardiac monitor that Mrs. Brown is in PEA (Pulseless Electrical Activity) and commenced cardiac arrest procedures.
For today’s lab we are going to break into two groups and practice what is required from the registered nurse in a cardiac arrest including:
• Correct cardiac arrest notification i.e. arrest buzzer/bell and telephone call
• Commencement of BLS
• Arrest trolley
• Cardiac arrest –the RN’s role
